캐나다 물리치료사 되기 (3) 실기시험 Ontario Clinical Exam

PCE에 통과했다면, 이제는 일하면서 거주할 주에서 담당하는 실기시험을 볼 차례입니다. 

 

한국에서는 보건복지부가 전국의 물리치료사 면허를 담당하고 있다면, 캐나다에서는 각 주마다 College가 있어서 주에서 관리하는 시스템으로. 제 경우 College of Physiotherapists of Ontario(CPO)에서 실기시험을 주관하고 있습니다.

 

시험은 크게 두 파트로 나뉘고 총 3시간 동안 온라인으로 진행되는데, 인터뷰 형식으로 환자를 대상으로 이야기하지 않고, hands-on으로 평가되는 건 하나도 없습니다. 시나리오가 주어지면 어떻게 할 건지, 왜 그렇게 생각했는지를 적절하게 서술해야 하는 게 포인트인 것. 

 

지난 필기시험에서 도메인으로 나왔던 부분이 똑같은 구조로 나오는데, Essesntial Competency Profile이라고 치료사가 되기 위해 어떤 요소들이 필요한지를 나눠놓은 거라고 보면 됩니다. 

 

시험의 구조는 총 2파트로 환자를 어떻게 보는지에 대한 부분과 ethics 관련한 상황을 어떻게 대처하는지를 확인하는 형태로 나뉘어져 있습니다. 

 

파트 1

첫 번째 케이스에서는 다음과 같이 치료 전까지의 상황이고 

1. Accepting the patient (assess personal knowledge and appropriateness for physiotherapy) 
2. Informed consent
3. Clinical Exam
4. Clinical impression and recommendations

두 번째 케이스에서는 치료 계획부터 어떻게 끝낼지에 대한 부분을 이야기하면 되는거죠. 

1. Developing a treatment plan
2. Developing goals, patient collaboration
3. Collaboration, consultation, and referral to others 
4. Monitoring, reassessing, and modifying the plan, and/or promoting self-management
5. Discharge planning or transitioning care

두 문제의 세팅(hospital, rehab, LTC, home care, etc)과 영역(근골격계, 신경계, 심호흡계 등)이 다르겠지만, 적어도 한 문제는 rehab or private practice에서의 근골격계 환자 케이스가 나온다고 합니다. 

 

파트 2

CPO 홈페이지에 가보면 치료사로서 지켜야하는 윤리적/전문적 기준들을 확인할 수 있습니다. 이 중에 11가지를 두고 문제 상황을 해결할 수 있는지를 보겠다는 거죠. 어떤 상황을 제시하고, 지금 어떤 부분이 문제인지, 물리치료사로서 어떻게 대처해야하는지를 설명하는 능력을 보는 거라 예상합니다. 

1. Supervision and assigning care to support personnel (PTAs, Rehabilitation Assistants)
2. Expectations regarding fees, billing and account oversight
3. Addressing discrepancies between employer expectations and professional standards 
4. Conflict resolution
5. Confidentiality and privacy 
6. Infection control and prevention
7. Professional boundaries
8. Ethical issues
9. Prioritizing care and time management
10. Physical and emotional safety of the patient
11. Communication strategies
